New Patients!
Your first visit will consist of an exam with
Dr. Jenkins, possible x-rays, and an explanation of your diagnosis and treatment options. During this appointment we will gather information about you and your dental needs so that we may better meet your expectations.
Please bring with you the following:
- Picture Identification
- List of Prescriptions
If you have any medical conditions that might lead to special arrangements, please inform our staff of this information so we may be prepared for your visit.
We recognize the value of your time: For your convenience, we have patient forms available for you to download, print, and complete prior to your appointment. Please select the appropriate forms and bring them completely filled out to your appointment. If you have any questions about the information being requested, please feel free to give us a call.

We also work hand in hand with you and most major insurance companies in order to maximize your insurance reimbursement. Please make sure to bring your insurance information with you so that we can expedite this process.
